Key Coverage Criteria

Malpositioning of the upper eyelid, including dermatochalasia, blepharochalasia, blepharoptosis, brow ptosis, or eyelid injury.
Glaucoma or glaucoma suspect status requiring evaluation or monitoring.
Diabetic retinopathy and/or hypertensive retinopathy.
Macular pathology, including macular degeneration.
Functional assessment of the optic nerve for conditions such as papilledema, coloboma, drusen, optic nerve pallor, pseudopapilledema, Foster–Kennedy syndrome, optic nerve sheath hemorrhage, or optic nerve hypoplasia.
Intracranial hemorrhage, intracranial mass, or clinical concern for increased intracranial pressure.
