Response to Comments – DL36125 – Breast Cancer Assay: Prosigna Comment: Multiple commenters supported the coverage of Prosigna, an FDA approved molecular assay which can be provided in local qualified laboratories. Response: CGS and the MolDx contractor acknowledges the letters of support for coverage for this policy. Comment: A commenter asked to include Bill Types for independent laboratories. Response: Per CMS Internet-Only Manual, Pub 100-08, Medicare Program Integrity Manual, Chapter 13, §13.1.3 LCDs consist of only “reasonable and necessary” information. All bill type and revenue codes have been removed.
